Selective Mutism (SM) Support for Children and Teens

Does your child speak comfortably at home but go silent at school or in public? Selective Mutism (SM) is more than shyness—it’s an anxiety disorder that typically emerges between ages 3 and 5, often when children begin school. Left untreated, SM can interfere with social, emotional, and academic development, making it hard for children to form friendships, ask for help, or participate in everyday activities. Many parents feel frustrated, helpless, or unsure how to support their child.

Signs of Selective Mutism:

  • Speaking freely at home but remaining silent at school or in public

  • Difficulty communicating with teachers, peers, or unfamiliar adults

  • Avoiding social situations where speaking may be expected

  • Trouble making or maintaining friendships

  • Inability to ask for help or engage in classroom activities

Dr. Macphee’s Approach:

  • Behavioral Techniques: Gradual exposure to feared speaking situations, with positive reinforcement, praise, and individualized rewards

  • Parent Involvement: Live coaching and guidance so parents can confidently support their child at home and in everyday situations

  • Brave Talking Exercises: Start with parent present, then gradually fade support as the child builds confidence

  • Generalization Across Settings: Therapy extends beyond the office—to schools, playgrounds, and community spaces for real-world practice

  • Practice Opportunities: Homework assignments to reinforce progress between sessions

  • School Collaboration: Staff training, classroom strategies, and push-in sessions to ensure continuity
